What Does It Cost?
PRISM is covered by Medicare and most commercial plans and is free or low cost for most patients. It is offered by hospitals and health systems across the country. If we are not yet partnered with your doctor, we have a self-pay program available with plans starting from $25/month.

You should expect to receive care from a specialist stroke recovery nurse 7 days a week. Your nurse care partner will review your blood pressure and health data on a daily basis and speak with you on a video call every week or as frequently as needed. If they detect any problems they’ll reach out to you within 24h and you can message them any time.

High blood pressure is a common risk factor for stroke, in fact 80% of second strokes are avoidable through blood pressure control. Unfortunately, most people have no signs or symptoms of high blood pressure. We will help you monitor this important warning sign and get the care you need if you are at risk.

Yes, blood pressure monitoring is important, but having someone you can reach out to any time you need is equally important. Your care partner will help you understand your injury better, set recovery goals with you, help you track progress towards your goals and assist with care coordination. They’ll also keep an eye on your mental health and general well being, making sure you are eating well, sleeping well and keeping active.

Physical, speech and occupational therapy are important tools to help you build back strength, regain mobility and independence. We can also recommend virtual seated exercise programs to help build cardiovascular resilience and core strength. In some areas we are also working to co-ordinate in person group exercise classes.
